If we do not contract with your insurance, we can provide you with a Superbill which you can submit to your insurance company for out of network reimbursement. Please check with your insurance to verify how much they will reimburse out of network. You would be responsible for paying at the time of service and any amount that is reimbursed would be sent directly to you by your insurance.
We accept debit or credit card payments for services. Many companies provide Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA) that can be used to pay for visits as well. We also offer financing through Advanced Care Card. Please ask us for details if you need financing.
We require at least 48-hour notice for cancellation. You will be charged the full fee for any cancellation of less than 48 hours. Excessive cancellations will result in termination of services. We require you to put a Credit card on file for out-of-pocket costs including any copay, deductibles, late cancellations, or any other fees not covered by insurance... Your card will not be charged until after your appointment. If you are using insurance, we will bill copays up front and then bill insurance prior to any deductible payments.
